HOW DO I...
OBTAIN A REPEAT PRESCRIPTION ?

If you require a further supply of medication and the doctor has agreed to this, please use the counterfoil, if you have one, and send your request by post, or put your request into the box in the pram park at the surgery. Please allow us 48 hours to process your request and enclose a stamped self-addressed envelope if you would like your prescription posted back to you.
You can register with www.patient.co.uk and then order your repeat prescriptions on-line. This same service can be used to book appointments on-line. You can download a form or collect one from the surgery and we will then generate a password for you to enable you to register.
There is a company called Pharmacy2u who, if you register with them, can arrange delivery of your repeat prescriptions on a monthly basis. You would need to collect a form from the surgery which you complete and send back to them, once this is done you would just need to call to arrange delivery. A lot of your local pharmacies offer a similar service.
You can only do one of the above as the system gets confused if there are online request for the same patient from different sources.

HOW DO I...
MAKE AN APPOINTMENT ?

All patients are requested to make an appointment.
We have a number of appointments available to book every morning, you need to phone at 8:30am to book one of these. They often fill very quickly and if all the appointments are full you will be asked to either book an appointment on another day (usually 2 weeks in advance) of call back the following morning. If you feel your problem cannot wait then explain this to the receptionist and she will call one of the Doctors to see if they can fit you in at the end of morning surgery.
In the evening, cases of extreme medical emergency will be seen after the booked appointments if they cannot wait until the following morning. Repeat prescriptions, certificates and routine work cannot be dealt with at this time.
If you would like to register to book appointments on-line through patient.co.uk you will need to complete a form and return it to the surgery, we will then generate a password for you to enable you to register. This will also enable you to order repeat prescriptions on-line. The form is available to download from this website or you can come in and collect one from reception.

HOW DO I...
OBTAIN A HOME VISIT ?

These must be requested before 10.00am unless it is an emergency. The receptionists will ask for details so that the doctors may judge the appropriateness of the request. Most examinations and investigations can be carried out more thoroughly at the surgery - it is in your best interest to attend.

HOW DO I...
OBTAIN TEST RESULTS ?

The doctor will advise you how long it will take to process your result at the hospital. It is the patient's responsibility to enquire about their results and further management if necessary.
Please phone for results after 2pm.

HOW DO I...
MAKE A COMMENT/COMPLAINT ?

If you have any comments, suggestions or criticisms to make about services provided by the medical centre, these should be made in writing and addressed to Dr Hayley Dawson.d
